Use of Arthroscopic Portals in Shoulder Surgery: Technique and Strategy
Arthroscopic portals are small skin incisions created with a surgical blade for transcutaneous passage of arthroscopic instruments. The use of shoulder arthroscopy solely for diagnosis is limited in modern orthopaedics, and currently, surgeons routinely perform complex procedures arthroscopically. When performing arthroscopic procedures, the surgeon must navigate around bony obstructions and vital neurovascular structures to view and manipulate deep tissues of the shoulder. In addition, the need to perform mobile assessments of the limb with the arthroscope placed deep within the joint as well as the demand for manipulative examination of deep soft-tissues compounds the importance of proper positioning and establishment of portals. Considerable educational, laboratory, and surgical observation hours are necessary for mastery of shoulder arthroscopy.
