Arthroscopic Techniques to Minimize Recurrence of Multidirectional Instability
Multidirectional instability (MDI) is shoulder instability in more than one direction, one of which is inferior. Patients with MDI usually do not have a history of traumatic injury. The term was first coined in 1980 to describe instability in the anterior, posterior, and inferior planes in a cohort of patients, most of whom were successfully treated with open inferior capsular shift. Nevertheless, MDI can be challenging to diagnose and treat. Most patients are treated nonsurgically with physical therapy, but arthroscopic suture capsulorrhaphy with or without closure of the rotator interval can be an option when nonsurgical management is unsuccessful.
