Rigid Flatfoot With Compromised Soft Tissues
The indication for surgery using the medial approach to triple arthrodesis is pain refractory to orthoses, bracing, and/or shoe wear modification in patients with severe, rigid hindfoot valgus deformity who are at high risk for lateral wound complications. A standard lateral approach originating from the distal fibula extending toward the base of the fourth metatarsal is a relatively horizontal approach and may result in significant wound tension on opposition because of the associated stretching of the lateral soft-tissues on deformity correction. In addition, some patients may have had previous lateral foot surgery, soft-tissue injury in the region of the sinus tarsi, or other local or systemic conditions that compromised the lateral soft-tissues. The isolated medial approach is indicated for patients who are at risk for wound complications with the standard lateral approach because of these considerations.
