The Open Bankart Procedure for Bone Loss, Revision Surgery, and Soft-Tissue Deficiency
Open Bankart repair is indicated in patients who participate in high-impact sports, for recurrent instability after arthroscopic stabilization, in patients with midsubstance capsular tears and anterior laxity, and for primary repair of bony avulsions. Open surgery is associated with more risks compared with arthroscopic stabilization. The primary concerns after any open procedure for shoulder instability are injury to the subscapularis, loss of external rotation, and increased incidence of osteoarthritis. This chapter reviews use of the open Bankart procedure in the context of two recurrent instability cases. Technical keys for surgical success and patient rehabilitation are outlined for each patient case, along with tips for avoiding potential complications and pitfalls.
