Rotator cuff tears are commonly seen in older patients. Although most involve the supraspinatus tendon, anterosuperior rotator cuff tears involving the subscapularis tendon, long head of biceps tendon, and supraspinatus tendon are seen often. While arthroscopic repair of rotator cuff lesions is the current standard of care, with this technique it is imperative to define all aspects of the tear, including involvement of the subscapularis tendon, long head of biceps, and rotator interval. Long head of biceps lesions are readily recognized and often treated concomitantly with tenotomy or tenodesis. Recognition of subtle subscapularis lesions requires careful assessment of the insertion site and recognition of the “comma sign.” This guide presents our technique for arthroscopic evaluation and treatment of an anterosuperior rotator cuff tear with biceps tenodesis.Quick Tips
