Management of Spring Ligament Insufficiency in Patients With Adult-Acquired Flatfoot Deformity
Adult-acquired flatfoot is caused by the failure of ligaments in the foot and, in the latest stage, failure of the deltoid ligament that supports the ankle. A key joint that is involved is the talonavicular joint, which is part of the triple joint complex (talonavicular joint, subtalar joint, calcaneocuboid joint). The deformity seen at the talonavicular joint in patients with AAFD is often a combined abduction and plantar flexion deformity resulting in pronation Reduction of the deformity of these three joints is particularly important because fusion of these joints, which together supply critical motion to the foot, is best avoided through this part of the foot. This chapter focuses on the repair and reconstruction of the spring ligament complex in patients with adult-acquired flatfoot deformity. With successful surgical reduction of the deformity and adequate support from the spring ligament, fusion of these joints can be avoided.
