This Instructional Course Lecture provides a comprehensive overview of the evolving field of metastatic spine disease management, which is a critical area in orthopaedic surgery. As the prevalence of cancer and the number of cancer survivors continue to rise, orthopaedic surgeons are increasingly confronted with patients who have metastatic spine disease. This lecture aims to equip surgeons with the latest diagnostic techniques, surgical procedures, and treatment planning strategies necessary for managing these complex cases.
At the conclusion of this course learners should be able to:
- Diagnose metastatic spine disease using traditional radiographs and advanced imaging studies, such as MRIs and CT scans, focusing on identification of characteristic signs of spinal lesions.
- Assess the appropriateness of surgical treatment versus radiation therapy in patients with metastatic spine disease by evaluating tumor radiosensitivity, patient health status, and potential surgical outcomes.
- Perform direct circumferential decompression and other stabilization techniques in patients with high-grade epidural spinal cord compression to preserve neurologic function.
- Implement multidisciplinary treatment strategies by coordinating care with oncologists and radiation therapy specialists to optimize the outcomes of metastatic spine disease management.
- Use prognostic scoring systems, such as the Tokuhashi score and the Tomita score, to predict survival and guide treatment decisions in patients with spinal metastasis.
- Integrate new surgical and radiation therapy techniques into clinical practice, enhancing existing protocols to improve patient care.
- Critically analyze and apply the NOMS framework for decision making in the management of spinal metastatic tumors to ensure a comprehensive approach that accounts for neurologic, oncologic, mechanical, and systemic factors.
